Collection of Information:
We may collect personally identifiable information (“PII”). PII is any information that can be used to identify a person, such as first and last name, email, home or business address, phone number, or protected health information (“PHI”). PHI is PII that includes certain additional data points, specifically it also relates to: (a) Your past, present, or future physical or mental health or condition, (b) the provision of health care to You, or (c) Your past, present, or future payment for the provision of health care, and it is created, received, transmitted, or maintained by the Company.
Currently, PII is only collected by the Company when You submit Your PII to us through form submission via our Site or via email communication. Typically, visitors to our Site can use our Site without giving us any PII. Any PII that the Company collects is via voluntary form submission by a user and is received by email, which has been sent by You, the user. Information that is sent to us via email or otherwise provided via our online forms may also be considered PHI. To the extent we receive PHI, we protect such PHI in accordance with the Health Insurance Portability and Accountability Act (“HIPAA”), as further described in our HIPAA Notice of Privacy Practices (NPP). Please note, we cannot secure any PII sent by email because such information can be accessed by other internet users. If You send us a question by e-mail, our use and disclosure of that information will be limited to the minimum necessary amount needed to respond to Your question. We do not compile PII from users commenting on the Site, asking questions of us via the Site, or making charitable donations via the Site.
Google Analytics
We may use a tool called "Google Analytics" to collect information about use of this Site. Google Analytics collects information such as how often users visit this Site, what pages they visit when they do so, and what other websites they used prior to coming to this Site. We use the information we get from Google Analytics only to improve this Site. Google Analytics collects only the IP address assigned to You on the date You visit this Site, rather than Your name or other identifying information. We do not combine the information collected through the use of Google Analytics with PII. Although Google Analytics plants a permanent cookie on Your web browser to identify You as a unique user the next time You visit this Site, the cookie cannot be used by anyone but Google. Google’s ability to use and share information collected by Google Analytics about Your visits to this Site is restricted by the Google Analytics Terms of Use and the Google Privacy Policy. You can prevent Google Analytics from recognizing You on return visits to this Site by disabling cookies on Your browser.
